Use of the GHC 9-1-1 supplemental information 9-1-1 service is made available solely for the use of individuals who live or work within the boundaries of the GHC 9-1-1 area subject to the terms and conditions contained herein and applicable laws. This supplemental information 9-1-1 service is for individuals to self-register supplemental information that they feel will enhance public safety entities ability to provide them service for emergency call situations.

The information associated with this 9-1-1 service will be made available to all participating jurisdictions and agencies within the GHC 9-1-1 area that receive 9-1-1 calls. However, GHC 9-1-1 cannot guarantee and is not responsible for whether the participating jurisdictions will view and utilize the supplemental information that the individual has entered during an emergency call situation.

The information provided by individuals is intended to be used solely for the GHC 9-1-1 supplemental information 9-1-1 service to enhance available information in the event of emergency call situations. To the fullest extent provided by law, GHC 9-1-1 will not share or distribute the personal information gathered by this form except for the purpose of allowing participating jurisdictions to receive the information in the event of an emergency call situation, including a text request to 9-1-1 service or default short code when and if available. Your information is submitted over a secure, encrypted connection for your protection.

To the extent that an individual is submitting medical information or personal privacy information, the individual is responsible and acknowledges that while GHC 9-1-1 will inform the participating jurisdictions on the confidentiality restrictions associated with this information, GHC 9-1-1 is not responsible for confidentiality compliance by the participating jurisdictions once an emergency call or situation has occurred.

Depending on the circumstances associated with a particular emergency call or situation, after the emergency call situation the information provided may be confidential information under several laws, including, but not limited to, Texas Health and Safety Code Ann. Section 771.061, Texas Medical Privacy Act, HIPPA, and Texas common law privacy.  Please be aware that except where confidential by statute or common law personal privacy when you provide information to any government entity, that information may potentially be subject to the Texas Public Information Act and applicable federal legislation. Information about the Texas Public Information Act is available from the Office of the 
Attorney General.  Each governmental body must be contacted directly about the records they keep and their use of personally identifiable information.  To ensure that the fullest confidentiality protection desired may be available to the individual after an emergency call situation in which personal privacy information of concern may have been recorded by the participating jurisdiction, the individual may need to contact and have the responsibility to contact or respond to notices from the participating jurisdiction to assert personal privacy concerns and other additional confidentiality exceptions from disclosure associated with release of the information.

The supplemental information you enter will be provided to the participating jurisdiction based on the phone numbers or address information you submit. To maintain accuracy, every six months a verification of data email will be sent to the email address you provided. You must verify all data in response to the verification e-mail or the information may be removed from this GHC 9-1-1 service system.

YOU ARE SOLELY RESPONSIBLE FOR MAINTAINING THE ACCURACY OF THE INFORMATION YOU PROVIDE. GHC 9-1-1 ASSUMES NO LIABILITY FOR THE ACCURACY OF THE INFORMATION PROVIDED TO THE PARTICIPATING JURISDICTION OR UTILIZATION OF THE INFORMATION BY THE PARTICIPATING JURISDICTION.

BY SUBMITTING THE REQUESTED INFORMATION, YOU ASSERT THE FULLEST EXTENT OF CONFIDENTIALITY AVAILABLE UNDER TEXAS LAW AND REQUEST THAT GHC 9-1-1 AND PARTICIPATING JURISDICTIONS ATTEMPT TO MAINTAIN CONFIDENTIALITY OF THE INFORMATION ACCORDINGLY SUBJECT TO THESE TERMS AND CONDITIONS.
